Cleanup of RtpTransportControllerSend. This CL simplifies a lot of code that can be cleaned up after the merge of RtpTransportControllerSend and SendSideCongestionController. In particular, the role of CongestionControlHandler is reduced to only handle the pacer pushback and stream pausing mechanism.
