Merging TransportFeedbackAdapter and SendTimeHistory. They were already tightly coupled, merging them makes the relations clearer. We also remove the kill switch for removing duplicate feedback events since there has been no need to use it. The potential to account for bytes sent in AddNewPacket was also removed since it is not used by TransportFeedbackAdapter.
